Volunteer Forms
In order to start volunteering at the clinic, you need to have the following forms printed and filled out when you come to the clinic on the first day. The checklist will help you keep track of everything. If you will have a TB test (PPD) placed or read at Stanford: Print out the Stanford Hospital signature form and immunization record.
